Occupational Health Offers for General Practice Teams

NHS Hampshire and Isle of Wight are partnering with Hampshire and Isle of Wight Healthcare Trust Occupational Health Service to offer a range of occupational health services to teams in General Practice.

1. New starter Health questionnaires

This service is to be used as part of the recruitment process.

Practice Managers have been issued with a unique login to the HIOWH Occupational Health portal which will enable them to send new recruits a link to an online new starter health questionnaire. The new recruit will complete the questionnaire, once submitted it will be reviewed by the HIOWH Occupational Health clinical team; the practice manager will be advised of fitness to work, any recommended adjustments and whether an immunisation or blood test appointment is required, if new recruits require any immunisations or blood tests, they will be contacted by the HIOWH Occupational Health Administration team and an appointment will be offered at one of their OH clinics in the HIOWH region.

All returned questionnaires will be treated in strictest medical confidence and will not be shared with the individual’s employer. The information contained on the questionnaire will form part of their Occupational Health record, which will be stored securely by the HIOWH OH service.

2. Vaccination requirements

To ensure you have access to the appropriate occupational immunisations to protect you against infectious diseases HIOWH OH will advise you which vaccinations and blood tests are required. If vaccinations or blood tests are clinically indicated an appointment will be offered at an OH clinic near to you.

To access this service you are required to complete an online form https://solent.opasg2.com/selfreferral/ (customer code 4DQK75). You will be required to submit information regarding your job role and previous immunisation history. The form will be submitted directly to the HIOWH OH service, if you require vaccinations or blood tests an appointment will be made for you. If you are unsure about your immunisation history please check with your previous OH provider, the NHS App or your own GP.
